    A Complex Systems Approach to the problem of chronic infection: Although many acute infectious diseases are now effectively controlled by vaccination, we currently lack vaccines for many chronic infectious agents. These infections are of particular importance in the developing world, where hundreds of millions of people each year are affected by chronic infections such as HIV, tuberculosis and malaria. The Complex Systems in Biology group aims to understand the basic dynamics and pathogenesis of these infections, how the immune system interacts with them, and ultimately how we can control them. To achieve this we apply computational and mathematical approaches to understanding and predicting the complex interactions between host and pathogen. These interactions range from the molecular level (how immune molecules such as the T cell receptor interact with viral molecules), through to the dynamics of infection within individual hosts, and finally to the level of the host and pathogen population. The group brings together medical biologists, bioinformaticians, physicists, mathematicians, engineers and computer scientists to tackle these fundamental questions. The laboratory has a wide range of collaborations with experimentalists working in different areas of infection and immunity.     Disulphide bonds are covalent links between pairs of cysteine amino acids. All life forms make this bond. Human cells, for instance, make about 3,000 different proteins that contain disulphide bonds. These bonds have been generally considered to be either structural or catalytic. Structural bonds stabilise a protein while catalytic bonds mediate thiol-disulphide interchange reactions in substrate proteins. There is emerging evidence for a third type of disulphide bond which can control protein function by triggering a conformational change when it breaks and/or forms. We have called these bonds ‘allosteric disulphides’. We have predicted that about one in fifteen of all structurally determined disulphides is a potential allosteric bond, implying a significant role for these bonds in controlling protein function. Viral, bacterial and human proteins have been shown to be controlled by allosteric disulphides. Our research focus is to characterise control of important proteins by allosteric disulphides, be able to predict allosteric disulphides in proteins, and to exploit allosteric disulphides for drug development.     We are interested in several areas of paediatric infectious diseases. Our current research studies include the folowing:

    1. Congenital cytomegalovirus infection (cCMV):To answer the question of how common cCMV is in Australia, we are currently involved in an ongoing prospective national surveillance study on cCMV via the Australian Paediatric Surveillance Unit (APSU). Data collected include the incidence of cCMV, spectrum of disease manifestation and maternal characteristics. <br><br>
    2. Non-tuberculous mycobacterial infections in Australian Children. The incidence, spectrum of illness and clinical management of this infection in Australian children is unknown. The answer this, a prospective national surveillance study via the APSU is underway. Data will help us to understand how common this disease is in Australian children and guide clinical management of the condition. <br><br>
    3. Perinatal hepatitis C exposed infants. Only about 5% of infants born to hepatitis C infected mothers become infected with the hepatitis C virus. We are investigating immune mediated reasons for this postulating a cell mediated immune response by the baby clears the virus ealry in life. In addition, we are investigating the rate and type of decline of passively acquired hepatitis C antibodies in these infants. <br><br>
    4. Central venous devices and blood stream infections: We have designed a database to capture data on line related infections in children. We are have prospectively followed the trend in line infections with time after the introduction of a practice change based on recent evidence to minimise line infections (2% chlorhexidine). In addition, we are investigating the epidemiology of line infections at SCH and developing appropriate management guidelines for central line infections <br><br>
    5. Antibiotic Usage Surveillance. We are interested in monitoring antibiotic use at SCH with the aim of guiding rational antibiotic prescribing and impacting on antimicrobial resistance. We are investigating a tool for the audit of antibiotic use in children at this tertiary paediatric center 6) Acute bacterial meningitis (ABM) - we are interested in the epidemiology of acute bacterial meningitis in children. In addition, we have assisted in statewide guidelines on the management of ABM. Both areas are being studied at the present time.

    Emily Waller is the Research and Development Manager for the cross-faculty UNSW Initiative for Health and Human Rights. Her research interests include the links between health, human rights and gender in conflict-affected settings, especially in relation to international and institutional responses to sexual and gender-based violence. Her most recent work focused on the UN response to sexual exploitation and abuse by UN peacekeepers from a victim-centred and human rights perspective. She previously worked on a research project drawing on the experience of academics and practitioners to explore the interface between health systems, conflict prevention, support for post-conflict recovery and peacebuilding. This research has lead to the development of The Health and Peacebuilding Filter, which is designed to provide rapid assessment of peacebuilding and conflict prevention components of health initiatives in conflict-affected settings. Ms. Waller spent time in Sri Lanka and the Solomon Islands as part of the research team exploring the practical applications for the Filter.
