Cannabis Online Survey Consent

Before you complete the survey please read through the Participant Information Sheet below. Once you have read the information given you will have the opportunity to give your formal consent to participate by marking the box below.

All information that you submit is taken anonymously and you will not be identified in any way.



By going to the survey I confirm that I am over the age of 18 years and have read and understand the information provided in the Participant Information Sheet and wish to participate in the survey.

Take me to the online survey for people concerned about a family member or friend’s cannabis use.

Take me to the online survey for regular cannabis users (I have used cannabis at least weekly over the past three months).

Take me to the online survey for people in treatment mainly for cannabis use (I am currently in treatment or recently completed treatment in the last fourteen days).
