Rationale


Chronic disease such as diabetes are the biggest challenge to our health system. These diseases can be better improved by healthy lifestyles putting them into practice is a challenge.

OneSmallStep will be used to provide patients with advice to enhance these lifestyle changes.

Aims


The aim of this project is to design and pilot a web based program called OneSmallStep, that helps GPs and their patients manage ischaemic heart disease in partnership to improve ethe patients quality of life and reduce the cost of treatment.

Project objectives:

1. To understand the essential functions and features of web-based chronic disease management systems

2. To investigte the critical success factors affecting the implementation of such a system.

Design and Method


1. Perform a literature review of evidence regarding web based chronic disease management tools

2. Design a web based system focussing on diabetes that combines exercise advice for patients and clinical performance data collection from GPs.

3. Implement program in a pilot study of IHD patients.

4.Compare the impact of the various modules of the program on clinical outcomes.
